If the note falls within the plan of care (POC) dates, this error typically occurs if there’s a change in insurance (or insurance type) mid-treatment, or if the Initial Exam was started under a non-Medicare insurance type.
To Resolve the Error:

This typically happens when the appointment was cancelled or no-showed previously but then the status was manually changed back to check-in.
