As a Physical Therapist in outpatient practice for over 10 years, I have been frustrated with the lack of ingenuity and progress within our profession when it comes to documentation. When I graduated from PT school I was writing out my SOAP notes by hand, marking my billing sheets, and using dictation for evaluations and re-evaluations. A decade later nothing has changed.
